Coincidental penalties

Now I realise this may be a bit thick to all you hockey vets out there, but why are some penalties coincidental and some not? I know they have to be for the same thing (roughing etc) but sometimes the ref lets the teams play with five outskaters and sometimes with 4? I've tried to see a pattern to it but I can't. Someone please explain!!!
